Claims & financial intelligence for U.S. healthcare providers

Every claim, explained.

Payers use automation to adjudicate claims in seconds. Liquidity Health reads every remittance, prices every claim against payer contracts, then turns underpayments into revenue using an intelligence layer that learns payer behavior in real time.

Sits beside your billing software, not inside it

  • Works from files your billing system already exports
  • 837 claim files
  • 835 remittance files
  • CARC & RARC codes decoded
  • Nothing installed inside your EHR
  • No change to how your team bills

Follow the flow

Every claim, a clear path. Every payment, accounted for.

From the payer's response to your next step. Liquidity Health connects the claim, the contract and the payment—so nothing gets lost between them.

Three claims. Three paths. One payment bucket.

Why now

Payers use automation. Providers have billing offices that manually process claims.

Now that claims processing is automated, a denial can be issued in a second, at scale, by rules no one on the provider side can see. Contesting it is still a manual process. Most underpayments and denials are reversible, but most are never collected.

Their side

Decisions at machine speed.

Automated review issues denials and reductions faster than any billing office can read them. Every uncontested error is kept.

Predict

Predict How Every Claim Will Perform.

Every resolved claim strengthens your Claims Intelligence, revealing what each payer is likely to pay, how long payment may take, and which appeals are most likely to succeed. LH prioritizes your Work Queue by recovery potential and identifies issues to correct before the next claim is submitted.

Advance

Advance money you've already earned.

Once a claim's value and pay date is known, waiting ninety days is a choice. Draw against adjudicated claims and settle when the payer pays, in the same ledger.

Recover

Every Denied Line Explained. Every Underpaid Dollar Identified.

LH brings the payer's reason codes and a recommended next step together. Your team reviews the evidence, acts, and follows the claim through its next response. When a payer allows less than the contracted amount, LH identifies the line and the applicable contract rate. Your billing team can make the decision instead of starting another investigation.

  • Claims move through four stages as your team records what happened: New denial, Working / On hold, Resubmitted or Appealed - Payer pending, and Resolved.
  • A zero-allowed line properly bundled under NCCI is explained as bundled, not incorrectly flagged as an underpayment.
  • If a claim is denied again, it returns to the work queue in its next denial round, with its full history kept in the claim journey.

N382Remittance Advice Remark (RARC)

Missing/incomplete/invalid patient identifier

Something on the claim is missing, invalid, or internally inconsistent, so it could not be adjudicated as submitted.

Alert: You may not appeal this decision

N211: this decision may not be appealed. Correct and resubmit instead.

Recommended action

Read the accompanying remark code - it names the exact field. Correct that element and submit a corrected claim rather than an appeal.

In practice: The remark code (N or M) tells you what is wrong. Fix it and resubmit as a corrected claim, not an appeal.

Responsibility: Provider. Appeal: No - correct and resubmit.

The evidence

Every Finding Is Built to Be Defended.

An underpayment finding is only valuable if your team can prove it. Every contract shortfall is tied to the applicable , , , , and . When reimbursement is based on a percentage of Medicare, LH identifies the CMS fee schedule and locality used in the calculation. Your biller receives the evidence needed to support the dispute and strengthen the case for payment.

The Work Queue

Claims move when something actually happens.

  • New denial. Denial received and analyzed. Someone needs to act on it.
  • Working / On hold. Being corrected, gathering documents or preparing an appeal, or on hold.
  • Resubmitted or Appealed - Payer pending. Corrected claim or appeal sent. Waiting for the payer to respond.
  • Resolved. Fully or partly paid, upheld or closed, or transferred to patient responsibility.

Try it

Payers explain themselves in codes: CO-16, PR-1, N290. Each one has a plain meaning, an owner and a next step.

Something on the claim is missing, invalid, or internally inconsistent, so it could not be adjudicated as submitted. Here the remark names the field: the patient identifier.

You pay when money comes back. Not before.

Your team keeps doing the work. Liquidity Health explains each denial with a suggested next step, shows short payments with their arithmetic, and keeps every claim's stage in one work queue.

No. Your team keeps its billing system and its process.

Only for contract comparisons, where the rate lives. Denial explanations and short payments visible on the remittance itself work without them.
